About Dream
Dream Care Direction (DCD) is a Perth-based NDIS provider committed to delivering person-centred, reliable and compassionate support to people living with disability. We work with participants, families, support networks and other professionals to help people achieve their goals, increase independence and participate meaningfully in their community.
Our services include Support Coordination, assistance with daily living, personal care, community participation and support worker services. We tailor our support to each person’s individual needs, preferences, goals and circumstances.
Our Support Coordination approach focuses on helping participants understand and use their NDIS plans effectively, connect with appropriate service providers, coordinate supports, monitor progress and build greater choice and control.
DCD places strong importance on professional, respectful and consistent care. Our team understands that every participant is different, and we aim to provide practical support while promoting dignity, autonomy, safety and independence.
With a focus on quality service and strong communication, Dream Care Direction works alongside participants to turn their goals into practical outcomes and support them to live the life they choose.
