About Ephraim

Ephraim Care is an NDIS registered provider supporting people with disability, older people and their families across South West Sydney and Greater Sydney. We believe quality support begins with understanding, compassion and respect, so everything starts with what matters to you: your goals, your routines and how you like to live.

Our directors bring more than 15 years of hands on experience across disability, aged care and mental health. As carers themselves, they know what it means to trust someone with the people you love. That understanding shapes how we listen, how we plan support with you, and how we turn up every day.

NDIS supports: personal care and help with everyday routines, support workers for daily living, skills and independence, community access, social support and companionship, transport to appointments, shopping and outings, meals and meal preparation, support coordination to help you get the most from your plan, and Supported Independent Living (SIL).

Aged care at home: support for older people living at home, including those with Support at Home funding. Help with everyday activities, personal care, meals, companionship and social connection, and getting out and about, all built around your routines, comfort and wellbeing.

Getting started is simple. Tell us what you need and how you would like to be contacted. We talk through your goals and preferences, confirm what suits you and when support is available, then agree on the details together before anything begins. You can involve a family member, carer or anyone you trust at any stage.
