What is Change of Circumstances?

A form you submit when something significant changes in your life or support needs, which may lead the NDIA to review your plan.
If your situation changes in a way that affects your support, for example a new diagnosis, a hospital stay, a change in where or with whom you live, or a carer no longer being available, you can lodge a change of circumstances. The NDIA looks at whether your plan still fits, and may adjust it. Providing clear evidence of what changed and why it matters helps the request move faster.
